Field Trip and Activities Permission

Child’s Name:

I grant permission for my child to participate in the neighborhood walks or field trips in an authorized vehicle. I understand that I will be informed of all planned trips and that I may withdraw my permission for a planned trip if I so desire.

• I grant permission for the use of voice, video, image or likeness in photographs and/or video of my child to be included/used. The permission for use of any of the media listed below is allowed for the following:

Newsletters
Business Flyers
Facebook, Instagram, YouTube , Social Media
Company Website
Video by a third party (such as filming for a television commercial)
Outgoing messages on answering machines and/or voice mail

I grant permission for my child to participate in the activities and in the use of the equipment at the center.
